Why Does My Sciatica Keep Coming Back?

If you’ve been dealing with sciatica for weeks, months, or even years, you’re probably asking the same question almost every patient asks when they come into our office:

“Why does my sciatica keep coming back?”

It’s a frustrating cycle.

Your leg pain gets better for a while.

Then it returns.

Maybe you’ve already tried medication, physical therapy, chiropractic care, stretching, injections, massage, or simply waiting for it to heal on its own.

If that’s your story, you’re not alone.

One of the biggest reasons recurring sciatica doesn’t improve is because the treatment focused on reducing your pain instead of identifying why the sciatic nerve continues to become irritated.

At Body First Chiropractic, our goal is to find and correct the underlying problem—not simply provide temporary relief.

Sciatica is a Symptom, Not a Diagnosis

Many people think sciatica is a diagnosis.

It isn’t.

Sciatica simply means the sciatic nerve is being irritated somewhere along its course.

The next question is:

What’s causing that irritation?

For some people, it’s a lumbar disc.

For others, it may involve spinal joints that aren’t moving properly.

In many patients we see, restrictions in the muscles, connective tissues, and joints around the posterior hip can also contribute to ongoing irritation or reduced mobility of the sciatic nerve.

That’s why every patient deserves a thorough examination before treatment begins.

Why Does Sciatica Keep Coming Back?

In our experience, recurring sciatica often happens because one or more underlying problems have never been fully addressed.

Common reasons include:

  • The original cause was never identified.
  • Treatment focused only on reducing pain.
  • Joint restrictions were never corrected.
  • Muscles around the hip remain tight and restricted.
  • The sciatic nerve is still being mechanically irritated.
  • Patients returned to activity before normal movement was restored.

When these issues remain, the symptoms often return.

The Posterior Hip Is Often Overlooked

One area we frequently evaluate is the posterior hip.

The muscles and connective tissues surrounding the sciatic nerve—including the deep hip rotators and gluteal muscles—can become restricted and contribute to persistent symptoms in some patients.

If these restrictions are never identified and treated, the nerve may continue to be irritated during everyday activities.

This is one reason why some people experience recurring sciatica even after other treatments.

Not every case of sciatica originates here, but it is an important part of the examination because overlooking it can leave a significant contributor untreated.

What I Commonly See In My Office

What I commonly see in my office is a patient comes in and has had their Sciatic pain in their posterior hip and down their leg for months or years. They tell me that they have had all kinds of treatments, including PT, injections, medications and that they don’t want surgery. What I commonly discover when going through the patient’s history is that they have never done any of the treatments to actually directly treat the stuck Sciatic nerve, especially in the posterior hip. I will then be able to palpate the stuck nerve in their hip and I know that I can release where the nerve is stuck and really help their ongoing problem.

Our Treatment Approach

Every patient is different.

Rather than giving everyone the same treatment, we first determine what structures appear to be contributing to your symptoms.

Depending on your examination, treatment may include:

The goal isn’t simply to make the pain quieter for a few days.

The goal is to improve how your body moves so the irritation is less likely to keep returning.

Why Accurate Diagnosis Matters

Patients are often surprised to learn that two people with nearly identical leg pain may require very different treatment.

That’s because sciatica is a symptom with multiple possible causes.

A careful examination helps determine whether your symptoms appear to be related to:

  • A lumbar disc
  • Spinal joints
  • Posterior hip restrictions
  • Reduced nerve mobility
  • Or a combination of factors

Understanding the “why” behind your symptoms is one of the most important steps toward lasting improvement.

When Should You Seek Help?

If your sciatica keeps returning…

If you’ve tried physical therapy, medication, injections, or other treatments without lasting improvement…

Or if you’re simply tired of wondering when your next flare-up will happen…

It may be time for a different approach.

Our office focuses on identifying the underlying cause of recurring spine and joint pain so treatment can be directed toward the structures that need attention.
